 Prior to treatment at West End Orthopaedic Clinic, the Workers' Compensation patient should provide:

  • Correct spelling of patient’s name, address, phone number(s), date of birth and social security number.
  • A copy of the patient’s health insurance card in case the carrier denies the claim a bill may be submitted to their health carrier until issues are resolved.
  • Employer’s name, address, zip code, phone number and contact person.
  • Referring doctor or referral source.
  • Copies of medical records if treated somewhere other than West End Orthopaedic Clinic.
  • Specific date of the injury and the body part injured. 
  • Knowledge of whether or not a panel of doctors was offered to choose from.
  • Name of the workers’ compensation carrier, the adjuster’s name, and telephone number. 
  • Workers’ compensation claim number.
  • Attorney’s name and telephone number [if applicable].
  • If a Federal employee WEOC will need to know if:  patient has filled out paperwork and have filed it to the Department of Labor [DOL].
  • If a Federal employee WEOC will need to know if:  claim has been accepted and if a DOL WC claim number has been assigned.
  • If a Federal employee WEOC will need to know if:  patient has received a letter from DOL giving them their accepted diagnosis code(s).
